Street Health Team
Our Street Health Team is a vital extension of Bevan’s inclusion health services—working beyond the walls of the GP practice to deliver care directly to individuals where they are. This overarching service brings together a multidisciplinary team of nurses, paramedics, and support workers to provide clinical and wellbeing support to people experiencing homelessness and other vulnerable groups.
The team operates through a flexible and responsive outreach model, using our mobile health unit (the Bevan Bus) and working across partner agency settings, hostels, drop-ins, and temporary accommodation sites. Our focus is on delivering high-quality healthcare interventions in environments where traditional services may be out of reach for many.
We regularly attend local community drop-ins to offer in-person support, working alongside homelessness charities, housing services, and faith groups to identify those most in need. Whether it’s providing wound care, health checks, support with prescriptions, or simply building trust, our team ensures that people experiencing homelessness are not left behind in accessing vital health services.
The Street Health Team plays an integral role in Bevan’s joined-up approach, working closely with our GP practice, Wellbeing services, and partner organisations to offer consistent, compassionate, and coordinated care across Bradford.

Beyond clinical support, the team runs a variety of group sessions and wellbeing activities designed to help people settle into life in the UK. These sessions not only provide social connection and emotional support but also help build routine and resilience—whether through peer support groups, orientation sessions, or recreational activities like football and arts-based workshops.
