Street Health Team

The Street Health Team in Calderdale is a newly established service designed to bring healthcare and wellbeing support directly to those who are often excluded from mainstream provision. While we do not operate a mobile clinic in this area, our team is embedded within the community and works in partnership with other services and accommodation providers.

This multidisciplinary team—including nurses, paramedics, and support workers—offers street-based and hostel-based clinical support, building trust and delivering vital healthcare to people experiencing homelessness and other forms of exclusion. The team works hand-in-hand with our hotel-based services to create a wider, integrated offer that reaches all inclusion health groups in the area. Together, they ensure that individuals receive compassionate, responsive care wherever they are.

Our Migrant Health Teams provide responsive health and wellbeing services for vulnerable migrants housed in temporary accommodations across Yorkshire. Services include initial health screenings, mental health assessments, facilitation of access to primary care, and education on navigating the NHS. The teams collaborate with third-sector partners to ensure comprehensive support for migrants.

Interim Accommodation – Calderdale

Our Interim Accommodation Support Team in Calderdale delivers essential health services to asylum seekers and others housed in temporary accommodation. The team plays a key role in welcoming and supporting individuals and families as they begin to navigate life in the UK.

This multidisciplinary team—comprising nurses, healthcare assistants, support workers, and occupational therapists—provides initial health screenings, New Patient Health Checks, and coordinated ongoing care. Working closely with the local GP practice, the team ensures new arrivals are properly registered and have access to the full range of healthcare services.

In addition to clinical care, the team facilitates a range of group activities and wellbeing sessions to support integration, reduce isolation, and promote resilience. Activities include peer support groups, orientation sessions, and informal recreation such as football and creative sessions, giving individuals a safe and supportive space to connect and recover.
